Mike Johnson (Master of Mathematics and Misunderstanding) claims that this is due to a common problem found in the early version of WordPerfect used by Thomas Jefferson. What he actually typed was "All Men are CreMated Equal ... ".
Now it makes perfect sense and it is self-evident. Mike suggests that it should not be surprising that a people who would accept the nonsensical version without question for two hundred years should also have so much misunderstanding of "A well regulated Militia ..."
